Here's a 5' long shark that was washed up on the beach in July, 2013.  Folks here said it was a salmon shark. There might have been a story in the SC Sentinel.

Also, I'm down to hit up Bean Hallow with you, Bushy!  I tend to go during the week before work, if you are up for that.  Bean Hallow is not too far from Boulder Creek.

We are actually hitting Bean Hollow on Saturday FYI if you want to tag along. Caught a 40-41" halibut there last year. Meetup sometime around 7-8am

Ahhh I would nut I got plans. Wife and I heading to a friends house for the weekend. Won't be able to make it.  Next times I'm on the water will be this Friday SWS... then maybe a spur of the moment next week... Mon possibly. After that its MBK Tourney... I will be there.  Kayak connection has tourney in SC couple weekends after.  Trying to hit the water as often as the wife and work allow it before the baby comes.

What an incredible experience. Wow!

I saw a finning GWS outside the Capitola kelp beds about 10 years ago. One of the most powerful experiences I've had. Got within about 20' of it. Skiff was 12' and shark was longer than the skiff. I cant imagine hooking one in a yak. Don't know if yours was a GW, but from your description it was definitely not a blue (they're skinny) nor a porbeagle (wrong coast). I'd vote for the salmon shark to ease my mind, but sounds very much like a GW.

The shark in that photo is a white shark, probably in S Africa or Australia looking at size and behavior. I worked on SE Farrallon island with the biologists there and saw many. 
Blue shark sounds right for yours. A 12 ft white shark would be nearly as wide as a car. I have never heard of them jump when hooked. Thats the Air Jaws move reserved for hunting, and even at that very rare for white sharks.
Blue shark sounds like a good bet. Another to look at is Soup Fin, as they are caught by anglers.
 I had my first Thresher experience (100+lbs?) a month back off Bolinas. Just as I set the hook, thing went crazy about 5 feet from the boat sending up a depth charge-like splash before nearly flipping me over. Shattered the slider in an instant and left me stupefied. It was like "what did I get myself into here?"
 Intense fishing moment indeed with a 12 footer I would imagine. Way to survive it and bring home a story you will remember better than a halibut.

from your description I would guess salmon shark 1st, then GWS, but "in the heat of the moment" they would appear very similar so it would be just a guess.
Either way that is a lot of toothy shark meat flailing around near a kayak. Way to keep your cool and and keep fishing :smt002
