On days on the ocean when it gets sunny and warm, inevitably I'll get pretty hot in my wetsuit by day end and develop this "heat rash" on my legs which looks bad but doesn't really bother me that much.  Anyone found a way around that wearing a wetsuit?

Nope.  I get the same rash, albeit not quite as much as your recent photo.  I have been consciously readjusting and making sure the wetsuit material over my ankles is “loose”.  Seems to have helped.  Last time out I did not have a rash. 🤔
We're gonna need a bigger boat!

Underlayers, pouring water down your wetsuit periodically and prep h have all helped me with this same issue

That butt rash cream is 40% zinc oxide - you can probably use it as a sunscreen as well Micheal. I'd use the other hand to apply it though :-)

You can also use Desitin creamyo which has a lot of Zinc Oxide. That's caused by heat/sun/sweat.

It's similar to PME (Poly Morphic Explosion) aka extremely bad heat rash. Read up on PME.

It sounds funny but more and more people are getting PME.

Not a lot of sunlight under a wetsuit. Per the NIH, you need direct sunlight for the “explosions”.

Personally, me thinks the rash is fungal in nature. Me also thinks neem oil might be a solution for prevention. Regardless of what causes the rash, neem oil can be used as a treatment. If the rash is fungal, and the butt cream works to prevent it, it may be due to the three petroleum product ingredients smothering the fungus. Pretty sure zinc oxide has no effect on fungus.

Not sure I’d be using butt cream for sunscreen without consideration of other ingredients. Zinc oxide mixed with other UV absorbing chemistry breaks down in sunlight and becomes toxic within a couple hrs. I think sunscreen is generally safe in the sun, don’t know about butt cream.

I have some sort of fungal rash that breaks out when I'm doing stuff that makes me sweaty or getting dirty and sweaty like cleaning out my garage.  My Doctor prescribed some expensive cream stuff that worked, but also mentioned that you can use dandruff shampoo as an alternative.  Since I had a bottle of selsen blue, tried it while showering for a few days and it worked!

I get the same thing when there is the slightest red tide.  The rash is worse when the red tide is bad.

How do you treat a red tide rash?
Scrub the irritated skin area with soap and water and flush it extensively with fresh water. Rinse with isopropyl alcohol (rubbing alcohol). Hydrocortisone cream may be applied 2 to 3 times daily to relieve itching.
Watching the river and itching for salt.
