I am sharing this here to assist the students and their project:

My name is Jim Snyder and I am a mathematics instructor at Anderson Valley High School.  As part of an ambitious Statistics project, two of my classes have put together a public opinion survey about a variety of social, political and current-event topics.  The goal is to gather data from our target populations: residents in the state of California, and a subset population of residents in Mendocino County.  The students will use the data collected to look for trends and correlations among demographic populations, as well as compare public opinion in Mendocino County with the rest of California.

OK so I took the whole survey, I wouldn't be surprised if many people objected to the amount of personal info they want, but without it the survey wouldn't get them what they need.

I'd like to see the results,

This kind of socio-geographic-economic opinion stuff really interests me.
I was born and raised in the bay area and know that has affected my thinking, I also know my opinions have changed over the years.

Personal information? It's a anonymous survey. The closest thing to personal information is what city you live in, and it doesn't ask for anything that can be considered PII (https://en.wikipedia.org/wiki/Personally_identifiable_information#Examples)
