My favorite is Hook1 Kayak Fishing Gear. They have very good prices and top-notch customer service. Last order, I accidentally requested the straight flush-mount holders, instead of the angled flush-mounts. When I called to ask about an exchange, the gal Karen said, “No problem, I’ll send out the angled ones to you today. Just return the straight ones when you get a chance.” The fact that she had faith in her customers gained my loyalty. Heck, my new angled holders arrived before I had even shipped back the old ones.

I have the Scotty rod holder extenders. Both the longer SS one and the shorter plastic one. I prefer the shorter plastic one.
Oh, and I think we also get a discount as members of NCKA.

fisherman's warehouse in manteca also carries them.  but you'd have to drive there.  they support our sport (eric kaii) but are not necessarily site sponsors.

Hook1 is the S#!T...Karen is great. I ordered some wheeleez from them and she tossed in a couple of extra pins. They ship insanely fast. I think I got an order two days after I placed it.

I did my part and kept it in the family. Stopped by the FW in San Jose and burned my entire lunch hour. I love that place and went with the intention of picking up a rod holder. Did that and picked up a new revo, some lures and a bunch of other cool stuff(oh my aching bank account). Glen is a cool guy and he even spooled up my reel for free!
