I'm putting together an order from digikey for some stuff.. I can add this jack to it. Pretty straightforward to desolder and remove the mangled one and put a new one in.

I usually remove the old stuff, make measurements or bring to electronics supply shop to get a matching replacement.

Theres a chance jameco or san mateo electronics will have it.
