the reason i ask is because the clothing order form says to put name and phone number on registration form but i don't see any registration form.

The clothing order form is also the registration form.  First payable line is for the registration fee, then the clothing items and costs.  Under those, is enough room to put your name and phone #.

That's what I did anyways; paid my registration via that form and added my name and phone #. 

Ahh, now i get it. Thanks Jerry.very stealth . maybe next year we can just download a blank piece of paper and write our names in disappearing ink.
