I think this has been discussed before and there seems to be many opinions as to whether wetsuit rash is caused by bad hygiene, allergic reaction or friction.
I started washing my suit more carefully and reduced incidents of rash. After 2 days crabbing at Crabfest I had very little or no rash. At Pyramid Lake I had Patagonia capiline thin thermals under my wetsuit but still got some rash. I had assumed that the extra layer would solve for it whether it be due to friction or bacteria. Then yesterday I got it really bad at Bullards Bar when I overdressed and was pretty warm in the farmer john. (see pic) It even left marks from my socks I had in my booties. So again an extra layer did not reduce it. Is it heat related? I've been sticking the suit in the wash inside out and it smells very fresh.
 
Some people have mentioned lotions or petroleum jelly to reduce friction but that seems like a very bad idea (and brings to mind that scene from Friends with Ross trapped in the bathroom trying to get back into his leather pants.)

Any insight would be appreciated.

I suggest you switch over to using a bib or drysuit. I love my Kokatat gear.

I use spandex bicycle pants,under the wetsuit. No rash,easy on easy off with the wetsuit.
We have the government, that our forefathers warned us about.

Dang, I didn't notice you had a Farmer John on. It was warm out there. You must have been roasting! I just had my splash pants over my jeans and was warm. Sorry you're suffering.

      I had the same kind of rash on my stomach.  I imagine it was sitting in my sweat. I had been giving the wetsuit a light wash with shampoo and drying it thoroughly in the sun.
      I started using this Revivex shampoo and the odor eliminator (separately) kneading the wetsuit in a 5 gal bucket of water. I was grossed out how much brown gnar came out of my suit and boots.  I also started wearing a thin underarmor shirt between me and the suit. Not sure if it's the best product or method, but I haven't had the rash issue since.

I think this is the fault of Capilene.  I stopped wearing Capilene under motorcycle gear because of similar rash/abrasion.  I've since switched to 100% merino wool base layers and haven't had that kind of rash/abrasion since.  I have been liking the Icebreakers brand 200 gram 100% merino stuff... I find the odd colors on sale on various outlet sites and buy them up.  If you prefer synthetics, I haven't had issues with other brands (REI's house brand is decent) -- this was specific to Capilene's weave in my experience.

May have been from bacteria.  I wore/wear spandex tights under the FJ every time. I've only had that type of rash three times all three were red tide launch/landings. 
Watching the river and itching for salt.
