More than likely a bad captain that doesn't know how to back up without flooding the boat.

Yeah - the captain was reported to be beginning a back down, slipped and fell, nailing the throttle in reverse which is how they ended up getting water over the transom and flooding the boat. Conditions were reportedly much rougher than the photos looked, too.
