It takes only seconds to make.

- Piece of paper - small enough to fit into your PFD pocket
- write down your name and the name of your emergency contact + phone number
- to water proof it, simply use clear heavy duty shipping / packing tape and apply tape on both sides and you are done!

This is such a simple thing that we often forget.
Just make sure it is actually on your person...we do this in cycling, and some folks put it on their bike, which is quickly forgotten about and never airlifted to the hospital with you...

Good point Steve. I have an E-card inside my PFD pocket and another taped underneath my kayaks hatch. Thinking of putting another one with my fishing license since I wear it around my neck.

side note: and on your cell phone shoud be your *ICE* number too (In Case of Emergency) if you're found on land in some type of emergency (heaven forbid) anyone can look at the *contact list* and call them.
