We've had several identity theft things over the past couple of years. In one case, somebody filed a federal tax return in my name claiming a big refund when we actually owed money. Of course, that was detected by the IRS. Our credit cards (not lost or stolen) have also been abused several times.

The form says that our data was apparently part of the breach, but how would there be any possible way to know whether a particular problem was due to the Equifax breach? Anyways, I filled out the form and will see what happens...

I believe it only asks for last 6 digits of the social security number. An enterprising cyber-criminal can probably get more useful info than that with a couple minutes of Googling...

The first four years of credit monitoring supposedly include all three bureaus.
